Established in 1995, LifeLinks is contracted by the Ministry of Health to deliver Outcome Planning and Outcome Coordination services for people aged between 0-65 years who have an intellectual, physical, sensory or neurological disability (or a combination of these) and their families/ wh¿nau/aiga/care givers. Our services are provided to service users/tangata whaiora and their families/wh¿nau who reside in Canterbury, South Canterbury and the West Coast.
Each year LifeLinks partners with nearly 5,000 disabled people and their wh¿nau to develop outcomes-focused personalised plans and facilitate their access to a tailored range of natural, universal and Ministry of Health-funded supports that contribute to their achievement of the outcomes they seek for their lives. There is no cost to you for receiving LifeLinks outcome planning and outcome coordination services.
Our services are designed to enhance service users¿ participation and inclusion within their communities and to maintain the wellbeing and resilience of their respective families/ wh¿nau/aiga/care givers.
